Supported Living

At Connect Into Care, our Supported Living service empowers adults with learning disabilities, autism, and complex needs to live independently while receiving the right level of support.

We provide safe, comfortable homes where people are encouraged to make their own choices, learn new skills, and live fulfilling lives—on their own terms. Our team works closely with each individual, their families, and health and social care professionals to create personalised support plans that reflect their needs, preferences, and goals.

Support is available 24/7 and can include:

• Personal care and daily living support

• Help with managing money and bills

• Support with medication and health appointments

• Community access and activities

• Developing independent living skills

• Emotional wellbeing and behaviour support

We believe in promoting independence, dignity, and inclusion. Whether someone is moving into their first home or transitioning from residential care, we provide the reassurance and continuity they need.
